Medicine Case





A 60 years old lady presented in Emergency Department with epigastric pain from last 4 hours.
She is a diagnosed case of Hypertension.

O/E
Pulse : 84
Temperature: 99 FH
B.P : 180/110.

ECG showed S wave in V2 > 1.5 mV
R wave in V5 > 2.5
and Q wave more than 3 small squares in V3

She was given oral NITROGLYCERIN ... Blood pressure lowered down to 130/85 ... and pain got relived unexpectedly. She was discharged on Ca channel blockers.

On follow up, her blood pressure was 150/100. Doctor changed her Medication to Beta blockers instead of Ca channel blockers.

After 2 days she again presented with sever epigastric pain and her blood pressure was 125/85.
